Scientific session research presentation] Today's health care provider must have easy access to evidence-based research in order to provide safe "best" patient care that looks at all facets of a person's condition; physical, emotional, psychosocial and spiritual. This interdisciplinary care must include the patient, family and/or caregivers. Rather than healthcare providers collaboratively building on one another's knowledge and expertise, caregivers provide care in a "silo" approach; thus the comment, "Don't you people ever talk to each other?? Through the use of a supportive professional practice framework consisting of evidence-based interdisciplinary clinical practice guidelines (CPGs) combined with an integrated documentation system, the healthcare providers will be clear on the disease pathophysiology, goals of treatment, potential physiologic problems, and specific interventions based on the potential/actual problems. A mutual education and evaluation plan (developed by the patient and healthcare providers) will support the patient in the best obtainable outcome(s).
