The purpose of this presentation is to challenge prevailing frameworks that guide practice development initiatives in nursing. The nature of nurses' work and what constitutes excellence in nursing practice is forever changing in response to innovations in technology, new knowledge, and evolving relationships with more informed consumers. To ensure that nurses respond to these change incentives in a timely and professional way, nurse leaders would be wise to attend to issues of worker motivation. Specifically, what are the factors that prompt nurses to discard old ways of doing things in the interest of adopting new practice patterns? A synthesis of the literature related to organizational development, knowledge transfer, change theory, evidence-based practice, and research utilization will be presented. The underlying suppositions of what motivates nurses towards practice excellence will be highlighted for discussion. Past approaches related to changing practice either implicitly or explicitly blamed the individual nurse for some deficiency. Either she lacked the knowledge, the motivation, or the attitude needed to provide excellence in care. While more recent change frameworks contest theories of blame and suggest that contextual factors contribute to our knowledge tranfer failures and successes, the concept of context is as yet undeveloped. A research proposal designed to help close this gap in understanding will be offered for debate. The study will utilize institutional ethnography, a method of inquiry that diverges significantly from traditional research designs. The goal of the research is to explore the relationship between knowledge and power. Decisions about what knowledge is legitimate and thus what knowledge nurses should adopt to change their practice will be the focus of study. This inquiry will contribute to the development of the concept of context as it relates to knowledge transfer and ultimately to our understanding of how to lead successful practice development initiatives in nursing.
