Objective: This study seeks to identify the main offence situations to the child's psyche during the service, so that, based on these data, be elaborated proposed for the improvement of the service of hospital and home, the child. Design: The child trauma victim, it cannot be, in any moment during the service, treated as an adult in miniature. It would not be any exaggeration, to consider that the service to the child trauma victim is, in function of their particularities anatomical, physiologic and psychological, more compound than the service to the adult victim of this offence to the health. Now many services of attendance to the health, modified their structures physics and operational, they qualified and they qualified their professionals to the pediatric attendance. Many professionals of the area of health abandoned the fear of treating children as children, and when necessary " they diffuse " in the lap, they hold them the hand, they play, they sing and they calmly talk, allowing to the small ones patient the best comprehension of the techniques and procedures to be accomplished. However, we still noticed that many services of attendance to the child and many professionals meet disable for the child's trauma victim service. Population, Sample, Setting, Years: For the accomplishment of the study 415 services were observed children victims of traumas serious, moderate and light from the entrance in the emergency room until the high of hospital or transfer for other unit, hospital or residence. The study was developed in the period from 1/08/00 to 31/01/01 in an emergency room in the city of Curitiba - Brazil. Methods: It is a descriptive-exploratory study, in which services were observed children trauma victims. Findings: Of 415 observed services, 93 (22,4%) they were accomplished at the emergency room. Of these 93, 57 (61,2%) they involved adults' concomitant service and children. In 77,6% of the total of services, the children were attended in hostile atmosphere and with little privacy. In 86 services there was the need of the suture accomplishment in atmosphere ambulatorial, and in 51 cases the child's physical contention was accomplished. Conclusions: Based on the obtained data, it can be concluded that the pediatric patient needs an individualized service and specialized. It is fundamental the creation of specific atmospheres to the children's service. Appropriate places are indispensable for the appropriate attendance, permanence and recovery of the small ones patient. It is careful, also, the use of methodologies attendances that respect the children's characteristics. These should be treated as child, this way it owes if it uses puppets, drawings, books and materials that are part of the daily infantile, doing with that these understand the procedures more easily to be accomplished by the professionals, minimizing the suffering and avoiding that negative implications happen to the infantile psyche. It is noticed sharply that the children that stay in the company of a family being, above all the mother, they sit down safer and receptive to the attendance, to the opposite they show anxious and agitated, evidencing the fundamental presence of the family during the pediatric service. The physical contention, frequently used, during the service the child trauma victim, they have more psychological implications certainly of what looks, being an outdated technique, and that it should be, urgently, substituted by options that turn the attendance to the less aggressive and traumatic child. Implications: The most important implication of this study is the perception on the part of the professionals involved in this work, that there are the training need and improvement of the techniques used during the assistance to the pediatric patients.
