21st INRC[Evidence-Based Practice Symposium Presentation] Removing barriers to behavior change has been well documented, and application of these theoretical constructs when developing and implementing evidence based practice in a community hospital can successfully assist in moving the process forward.  This presentation reviews the theoretical foundation for behavior change, allowing the participants to anticipate and remove the barriers while enumerating and illustrating the benefits.  Direct application of these theories to specific challenges faced at the Englewood Hospital and Medical Center's Center for Nursing Practice will be discussed, providing in-depth lessons learned.
