Background: Poverty has a profound effect on the health of families by negatively influencing access to health-promoting behaviors. Children and adults who live in low-income families are more likely to have poor health. Moreover, recent changes in the welfare system and healthcare financing in the United States have had a significant impact on the health care of low-income families. These families are unique in their perceptions of the everyday reality of health and illness that affects the choices they make to promote the health of their families. Although “family health” has recently received attention by nurse researchers, “family wellness” as a concept is not adequately addressed in the nursing literature. Many of the existing tools that are used to study family health are relevant only to Caucasian, middle-class families. A “contextual conception” of family wellness for culturally diverse low-income families is needed. Objective: The purpose of this qualitative doctoral study is to describe how family wellness is perceived by mothers in low-income families. Setting: The Head Start program is designed to enhance the educational preparedness and overall health of preschool American children in low-income families. Usually, the mother in the home is the health care decision maker for her family: therefore selected participant mothers will be interviewed. Methodology: The process that is being used to delineate family wellness and discover what it means to Head Start families is that of concept development guided by Schwartz-Barcott & Kim’s Hybrid Model. This model emphasizes the ordinary context of the concept enhanced by fieldwork that supports concept exploration. The three phases of the Hybrid Model are the theoretical phase, the fieldwork phase, and a final analytical phase. For this study, a multidisciplinary literature review during the theoretical phase will provide a working definition of family wellness as the basis for a semi-structured interview guide. During the fieldwork phase, participant mothers in Head Start families will be interviewed concerning their views of family wellness. Lastly, analysis of this qualitative research will compare and contrast the mothers’ views with the concept of family wellness inferred by the multidisciplinary literature. Implications: Concept development of family wellness, as it is perceived by Head Start mothers, will contribute to further tool development and inquiry as well as provide rigor for family nursing mid-range theory construction. This research will expand nurses’ responsiveness to the unique nursing needs of underserved low-income families with children. Furthermore, nurse educators will have an increasingly sound foundation from which to teach family nursing.
