Because of its involvement with entry-level licensure testing, the National Council of State Boards of Nursing (National Council) has developed an unparalleled body of knowledge about the competencies of nurses as they enter the profession; however, safe practice after the entry period has not been studied. Health care regulatory bodies in the US are being called upon to demonstrate that those under their jurisdictions maintain the ongoing ability to provide safe care to the public. The first step in assuring ongoing competency is the discovery of the characteristics of safe post-entry care. National Council has initiated a study of nursing practice in the US during the first five years post-licensure. The purpose of this presentation is to report findings from the first phase of the study. Objective: This study will discover not only how nursing practice in the US changes post-entry, but also when the change occurs and if the educational preparation of the nurse influences practice after the first six months. Specifically, this study will: determine the characteristics of post-entry nursing practice, identify the chronologic evolution of nursing practice, evaluate the contribution of internal and external factors to the evolution of nursing practice, and identify the determinants of safe post-entry practice. Design: During phase I, data will be collected using a cross-sequential design, which incorporates the aspects of both cross-section and longitudinal designs, to survey nine different cohorts of registered nurses. Sample, Method, Timeline: Each cohort will be selected based on years since initial licensure so that cohorts represent registered nurses who have been in practice 6, 12, 18, 24, 30, 36, 42, 48, and 60 months. In order to ensure at least 100 nurses in each cohort, 1000 registered nurses will be recruited from across the nation for participation in the study. Data will be collected during January and February 2002. Method: Each participant will be randomly assigned to describe activities selected for the study by a Subject Matter Expert Panel. Each selected activity will be assigned to a number of nurses within each cohort. Each nurse will be asked to supply a detailed narrative of their last day of work (i.e., how their time was apportioned, the focus or priorities for that day, the types of staff and clients with whom they interacted, etc) and a detailed narrative description of how they last performed their assigned activity. Data will be analyzed with a qualitative, grounded-theory methodology. An Advisory Panel, composed of NCSBN members, at least one member of the regulatory community outside of nursing, and nurse researchers with expertise in nursing education or practice, will be assembled for the study. Advisory Panel members will provide independent reviews of quarterly data summaries with feedback to the primary investigators about the overall value of the data for addressing the aims of the study. The Subject Matter Expert Panel will be composed of individuals (from all areas of the country and experienced in the major nursing specialty areas) that work with or supervise newly licensed, and post-entry nurses. The two primary investigators will independently read the narratives supplied by each of the nurse cohorts, noting changes in daily work and changes in performance of selected activities. Patterns of practice, issues related to patient safety, and changes in perceptions across cohorts will be discerned. Input from the Advisory Group and Subject Matter Expert Panel will be used to modify the questions asked of the longitudinal nurse cohort to best achieve data answering the specific research questions. Implications: The cohort of participants surveyed within the first 6 months of practice will then be followed for a period of 5 years. This study will enhance understanding of the characteristics of post-entry practice in the US, identification of internal and external influencing factors, changes occurring in practice over time, and differentiation between safe and unsafe practices.
