Focusing on patients not only demands innovative solutions to patient/family concerns, but also creative approaches to educating staff so they are able to embrace integrating patients and families as equals on the health care team. Aims of this project include improving patient and nursing satisfaction through patient and family centered work redesign that is timely, safe, spiritual and evidenced based; promoting a supportive and collaborative working environment through effective care teams that encourage new ideas and share decision making; achieving positive patient outcomes; and celebrating failures as well as successes.áEvaluation includes baseline and ongoing measures: time and motion studies; satisfaction surveys completed by members of all disciplines including nurses, nurse aides, physicians, physical therapists, respiratory therapists, and housekeeping; fall, pressure ulcer and adverse drug event rates; RN vacancy rate; staff turnover rate; patient satisfaction scores; and nursing hours per patient day. Examples of innovations include, but are not limited to the following: physical redesign of the unit; safety "huddles"; quiet zones; communication boards; and staff status boards to indicate whether staff need help or could handle additional patient care assignments. áOutcomes achieved include, but are not limited to the following: Fall rate decreased to 0; pressure ulcer rate less than 0.0040; no adverse drug events; RN vacancy rate remaining at 0; voluntary staff turnover rate decreased to 0; and increased patient satisfaction.
