Objectives for this paper are to 1) describe the content and use of four instruments for testing cognition in elders, 2) identify dimensions of cognition measured in the NDB model, and 3) identify methodological issues in the administration of tests to cognitively-impaired nursing home residents. Design: These tests are used as part of the measurement of the background factor of cognition in a trio of inter-related descriptive explanatory studies use the NDB model to explain wandering, aggression, and vocalization behaviors in residents with dementia. Sample: The wandering study sample to date comprises 59 subjects with dementia (forty-four ambulatory and fifteen non-ambulatory) from a projected sample size of 280. Concepts Studied: Wandering; Cognition: Memory, attention, visuospatial, and language. Methods: The Mini-Mental State Exam (Folstein, 1975), Severe Impairment Battery (Saxton et al, 1993), Functional Linguistic Communication Inventory (Bayles & Tomoeda, 1994), and the Arizona Battery for Communication Disorders of Dementia (1993) were piloted with three subjects. Test administration protocols were modified to address pilot implications: MMSE=11 became the cut-point for administering items from the ABCD to minimize ceiling/floor effects; subject burden was reduced by eliminating instrument overlap on language items; agreement was reached across projects regarding testing times to limit fatigue, testing place to enhance comfort, and acceptance of word variations to accommodate culturally-biased word representations of objects. Findings: The mean MMSE score (qualifying criteria of MMSE<24) for 42 residents is 9.81(Range 0 - 23). Three residents refused to cooperate with cognitive testing while 13 were untestable. Conclusions and Implications: Although the analysis of the select dimensions of cognitive function are incomplete, the severe level of impairment represented by the mean MMSE score alone suggests that targeting cognitive interventions may not be useful in the management of wandering.
