HeartCare is a computerized internet-based CABG home recovery program. HeartCare resources are delivered to patients through project-supplied home-based WebTV's and standard television sets. We sought recovery information specific to five aspects of each patient: patient health status, gender, co-morbidities, risk factors and recovery time frame. We evaluated 750 WWW pages of cardiac recovery information on the Internet for accuracy, appropriateness, reading level, and gender focus, and selected 350 for use. We created 160 pages of cardiac recovery information to supplement information not available on the Internet. These Web pages have been entered into a data base and are dynamically "pulled" according to a tailoring algorithm and supplied to patients at their home each time they access the HeartCare System. Organized into sets of menus, the content presented to an individual changes over four time periods: first two weeks post discharge, 3-6 weeks post discharge, 7-12 weeks post discharge and months 4 and beyond. We have demonstrated the feasibility of applying an information-tailoring algorithm based on patient information needs, and structuring this information to be easily retrieved from Web page menus that change over 6 months to provide customized teaching and home management support.
