Purpose: The health services literature, suggests Geographic Information Systems (GIS) as a useful tool for policy evaluation when location is an important factor in policy success. The research question was: "Was there a significant difference in nurse to population ratio change for health professional shortage area (HPSA) designated counties between 1991 and 2001?" Conceptual Framework: Nursing workforce distribution is an inherently local issue. Nursing shortages present serious concerns for local and state governments due to the negative constituent effects. The effectiveness of current nursing recruitment and retention policies needs to be examined. Subjects: Subjects were non-identifiable respondents of the RN Professional licensure survey 1991-2001 (aggregated by county). Method: Following IRB approval, the following Missouri data sources were used: County data, County HPSA designations (1991-2001), and RN Professional licensure data (1991 û 2001). Group differences (HPSA versus Non-HPSA counties) were explored using GIS data visualization, spatial statistics (K function, Moran's I correlation) and classic statistics (Descriptive, Mann-Whitney). Results: Results of both data visualization and statistical methods do not demonstrate a trend of decreasing group differences between HPSA and Non-HPSA designated counties. Only two of the five time periods studied had significant group differences. Between 1993 and 1995, the loss in nurse to population ratios in HPSA counties was significant (U=1020, p <0.001); however, between 1999 and 2001, the growth in nurse to population ratio changes in HPSA counties was significant (U=1032, p=0.001). Conclusions: The GIS data visualization and statistical techniques performed suggest that current policy definitions of underserved areas may not be effective in defining areas of nursing shortages and the existing policy implementation may not be achieving the desired effects.
