Advanced practice nurses (APNs) frequently use standardized languages to describe aspects of their practice, but rarely do they employ any of the standardized nursing languages. There are several models available for various types of data collection regarding standardized language in the APN educational setting. This Clinical Log (Version 4.5) has evolved from a paper and pencil format to web-based data entry and retrieval. The Clinical Log contains fields for the usual ICD, CPT, E/M codes but expands on other logs by including NANDA-2, NIC-3, and NOC-2 codes (which are linked) as well as the American Hospital Formulary System. The 4,000+ records currently available are analyzed for content and patterns including frequencies, trends over time (for individuals as well as cohorts), and common mistakes. All students use their data during the 4-semester clinical sequence for assignments integrated into the curriculum. AN: MN030382
