Purpose: To evaluate the reliability, validity, and feasibility of instruments measuring the physical demonstration and psychological perception of fatigue in laboring women. Theoretical Framework: Understanding labor fatigue is important in order to ascertain what care provider actions may increase/prevent fatigue. Subjects: n=29 primaparous women, over 18 years of age (m=29.4 years), planning vaginal singleton birth, without current treatment for pregnancy induced hypertension with magnesium sulfate. Method: Baseline fatigue evaluation at 37-40 weeks gestation, all applicable intrapartum measures i.e.: admission, first vaginal exam, beginning of 2nd stage pushing, infant scalp at the introitus, after the next to last push, before epidural analgesia, before forceps/vacuum extraction, before cesarean section, and finally within 24 hours postpartum. Self-report measures included visual analog (VAS) (Lee et al, 1990) and Likert scales (1-10 rating). An analog handgrip strength tester measured physical fatigue (Pugh, 1993). Postpartum survey evaluated protocol feasibility. Results: Satisfactory reliability was established between the VAS & Likert scales (r=.8), and between three repeated measures of the handgrip at each time point (r=.9). Negative correlation between handgrip and Likert in the intrapartum (r=-.598) demonstrates a moderate relationship between self-perception and physical fatigue. Participants rated protocol participation at a mean of 1.8 on a 10-point scale, (1=not difficult, 10=very difficult). Participant attrition was 24% (n=7) due to non-adherence, or self-withdrawal after labor began. Conclusions: Studies of the multidimensionality of childbearing fatigue (Pugh, 1993 & 1995) validated self-report and physiological measurement. Reliability and feasibility of such measures were supported in the current study and the intrapartum data extends their use into second stage labor. Future research should compare self-report/physical labor fatigue in specific populations, to compare spontaneous/directed pushing, epidural analgesia, and doula care outcomes. AN: MN030305
