Abstract:
Poster presentation, ANCC National Magnet Conference: Introduction of Palliative Care at the time a life threatening illness is diagnosed and support of the patient and family through the disease trajectory is the goal of all Palliative Care professionals. Inspired by a philosophy of caring and a novel delivery model already in existence in our Breast Health Center, this Poster presentation, ANCC National Magnet Conference« will share the ripple effect passion, commitment and nursing vision can have on healthcare. Leadership, communication and collaborative educational opportunities, combined with passion and a strong commitment to relieve suffering and enhance the quality of life for patients and their families facing life threatening illness, have been the cornerstones of success of this program. This program now serves as a model for others, serving over six hundred patients a year in our community. Strategic planning, lessons learned, obstacles as well as opportunities, will be outlined.
