Abstract:
Patient falls in hospital are reported to occur in the United States at 2.3 to 7 % for every 1000 patient days. Approximately 30% of these falls result in potential injury and 4-6% result in serious injuries and some result in nonfatal injuries and trauma-related hospitalizations. Fall related injuries account for a major portion of medical expenses for those who are 65 years and older. Hospital providers are worried about the risk management issues involved with the morbidity, mortality and financial burdens attributed to patient falls in the health care setting. With the need for continuous improvement on quality to prevent patient falls, the team conceptualized the project on Fall Reduction through Staff Education and Recognition in a health care facility of 326 bed not-for-profit hospital and conducted a pilot floor study on 24 RNs of the surgical unit. The objective was to identify concerns on patient falls, define patient falls and identify the need for assessment on fall reduction. With the help of The National Database of nursing Quality Indicators report and American Nurses association Quality and Safety Initiatives provided a systematic data collection on the fall risk assessment tools and staff recognition for success on Fall reduction rate. The methodology used for the study design is evidence based theoretical framework which has a systematic review policy and interventional plan for protocol on patient fall safety and education strategies, which results in pilot staff's attitudes, beliefs, values, and knowledge regarding patient fall prevention, assessment and reporting of patient safety practices. The intervention plan initiates education and skill building through lectures, discussions, and presentations on data compliance and protocols. The outcome measures were improved documentation, increased usage of patient fall protocol and assessments to achieve fall risk awareness and with the use of fall assessment tools to decrease the rate of patient falls. Study Design: This project involved the implementation of evidence-based practice change to improve nurse's knowledge and practice for fall prevention compliance (Figure 1). This study included a pre-survey to assess attitudes and beliefs regarding patient fall prevention goals, knowledge of patient fall protocols and awareness of patient safety practices. A retrospective chart audit at baseline prior to the practice change and after the educational interventions will be completed. Implementation of evidence-based intervention involved interactive activities or workshops consisting of case scenarios and role playing for adult learners to determine improve knowledge, detect skills, and change behavioral attitudes on patient fall prevention (OÆBrien et al. 2007). Figure 1.0- Patient Fall Risk Awareness Pilot Project Study Design Plan: 1) Evidence-based Theoretical Framework Meta-analysis and Systemic Review Policy and Interventions relevant for Patient Prevention, Protocol on Patient Fall Safety Compliance, and Education/valuing strategies; 2) Identify Pilot Floor Staff Attitudes, Beliefs, Values & Knowledge Regarding Patient Fall Prevention, Assessment & Protocols: a) Pre-survey knowledge-based assessment (Likert scale) of patient fall prevention, assessments, reporting & protocol on patient safety practices.
b) Longitudinal quality and benchmark data comparison sharing on patient fall compliance. 3) Evidence-based Intervention Pilot Project Education & Skill-Building

Patient Fall Risk Compliance. Surveys of nursing staff were conducted on the pilot floor for this CQI project. The surveys were distributed and collected by the unit secretary of the pilot floor. Anonymity was guaranteed to the participants. The survey assessment on nursing staff took place over a one (1) week time frame. Based on the pilot survey responses, the team determined that educational sessions facilitated and met the need of improving staff knowledge base on patient falls prevention.
