Purpose:

       Research purposes is to establish a case- managed model to integrate dementia patients’ continuous care networks, to provide early diagnosis, treatment and care needs, to relieve families’ concerns of the burden of medical costs. Through those, the dementia patients can get complete medical care.

Methods:

    In this study, the case management model involved integration of hospitals, foundations, long- term care provided by the department of dementia continuous care network, (1)Community care includes wheezing services, home services, family support groups, telephone counseling and family care services, (2) Institutional care includes dementia area and nursing home.(3). Medical services includes memory clinic ,especially for dementia outpatients, consulting services for outpatients, general acute elderly psychiatric inpatient services, and establishment of a complete network of medical care in dementia patients. 

Results:

     According to statistics from July 2011 to June 2012, monthly average among these months: (1). Community care: respite services for 45 people, home services for 48 people, the family support group for 5 times half a year, and family services through phone consulting for 45 times (2).  Institutional care: a chronic dementia word care  for 63 people, day care center services for 50 people. (3). Medical service: treatment of the dementia patients in outpatient service about 1093 people and care consulting in outpatient services for 203 people.

Conclusion:

    After the implementation of case management model, the dementia patients have 90.5% of the average rate of return visits. The case management model can enhance knowledge of the caregivers of dementia diseases and care-related skills, integrate dementia care networks to improve the quality of care, strengthen elderly dementia screening in the future. It also can held dementia promotion activities, such as family caregiver support seminars and train quality professional dementia care manpower and so forth.
