Abstract:
Session presented on Friday, July 24, 2015: In this symposium, we will present three case studies of patients treated for chronic hepatitis C while managing their co-morbid psychiatric illness in a primary care-based, interprofessional model. Each of our patient exemplars successfully completed treatment based on individualized care plans developed for them by our team which consists of a nurse practitioner, a social worker, and a patient navigator/care coordinator. Our care plans were based on the overarching goal of achieving a sustained viral response to treatment which included medication management, side effect management, crisis intervention, ongoing care coordination, and support. We used elements of Fonagy's approach to care, including inquisitiveness and flexibility to partner with our patients. Informing our treatment plans was a standardized, structured interview conducted with each patient on hepatitis C treatment readiness. Our care model has been extended to our wide patient population and has been duplicated in other practices offering care to similar patient populations. Our model of care can be adapted to serve other co-morbid patient populations.
