Abstract:

The Clinical Nurse Specialist is an effective agent in disseminating best practice regarding diabetes care using quality improvement processes in a primary care setting. A culture of attentiveness to diabetes care, work redesign, and sharing of provider adherence to quality metrics led to marked improvement in diabetes-related quality metrics.
